Imageboards in general are hard to moderate, there are a few techniques to change IP, and posting needs no registration. A dedicated autist with too much time on his hand can do lots of harm to those who just want to post and have their own fun.

Endchan's source is open, can be read and used, installed and tested. But by just opening a board anyone can have access to it.
